A Fellowship is a medical training period, in the United States and Canada, which a doctor or dentist may perform after completing a special training program (residency). During this time (usually more than one year), doctors are known as Associates . Fellows are able to act as Attending Physicians or Doctor Consultants in the field of generalists in which they are trained, such as Internal Medicine or Pediatrics. Upon completion of the Fellowship in the relevant sub-specialization, Doctors are allowed to practice without direct supervision by other physicians in sub-specialties, such as Cardiology or Oncology.

United States

In the United States, the majority of scholarships are accredited by the Board of Accreditation for Post-Graduate Education ("ACGME"). There are some programs that are not accredited, but are actually well received, given the importance of becoming a Board Certified Doctor in a major specialty, where Fellowship is often based more on research productivity.

    Combined scholarship

    There are programs that offer combined fellowships, training in two or more sub-specialties as part of a program.

    • Lung Care/Critical : this type of program is more common than the Lung Disease (non-combination) program. In 2007, there were 130 ACGME accredited lung/critical care programs while only 25 programs for Lung Disease alone.
    • Hematology/Oncology : in 2005, there were 125 ACGME accredited programs for Hematology-Oncology, while only 12 programs for Hematology alone and 18 for Oncology alone.
    • Geriatrics/Oncology : The American Board of Internal Medicine approves a 3-year joint fellowship program in medical oncology and geriatrics. John A. Hartford Foundation initially funded 10 institutions for this type of training.
